When I wished to refine a mask detail I was at first stumped by the smallest brush size being too large for the detail I was editing.
Found the solution or work around.
Go to the part of the picture you are masking and adjust the brush to the smallest size. If you need a smaller brush enlarge the picture size Ctrl ++ . The brush size gets smaller in relation to the picture hence a smaller brush for masking.
Also using the \ key (windows) shows the mask and I found that even when the red mask is showing you can use the focus and restore brush. When the brush stroke is finished move it away and after a delay the invisible amendment will show. Brushing on the red mask makes it easier to refine as you can clearly see what is needed.
